  • Location of patient (Facility)

  • Patient’s name and date of birth

  • Patient’s room number

  • The study needed

  • Signs & Symptoms warranting the exam (ICD-10)

  • Patient's condition supporting portable necessity

  • Your name and direct contact information

• You will be prompted to fax the face sheet and physician's Order

• Once we receive request documents a technologist will be dispatched to complete the exam

• If requisition has not been received via fax within 15 minutes of order placement please call to let us know

• Upon arrival the technologist will check in at the nurses’ station for the paperwork and then proceed to the patient’s location to conduct the study
